1.	The form has eleven sections and six pages in total.
2.	Please note that all mandatory fields have an asterix sign(*) against them.
3.	There is a 'Save' button on each page. If required, you may log out without filling up the form completely and re-login.
4.	Please click on the 'Submit' button given on the last page once all information has been filled.
